Hasbro Parts Order Forms

Please select the appropriate form for the product that needs replacement parts. Print out the form, select the parts needed, fill out the personal information, and fax or mail the order to Hasbro Direct.

1. Select the product for which you need a replacement part.
2. The appropriate form will display in a separate browser window.
3. Print the form (select Print from the File menu).
4. Close the second browser window.
5. Fill out the form and mail or fax it to Hasbro Direct.
